We are trying to improve our patient portal utilization. Please take a moment to activate your patient portal and review your medical record. If you need assistance, we can help you the next time you are in the office. Upon arrival, ask for a temporary user name and password. There is a computer work station set up in the lobby that you can use while you're waiting to be seen or after your visit. We're here to help!

Typically, it will be reviewed and someone from our office will contact you in 24-48 hours. Recall of Accu-Chek* Aviva Plus Test Strips.
On May 15, 2018, Roche announced a voluntary recall of some lots of Accu-Check Aviva Plus test strips because the recalled strips had cracked reagent.

Fluticasone Propionate Nasal Spray by Apotex
On 31 May, 2018, Apotex announced a voluntary recall of one lot of fluticasone propionate nasal spray because some bottle were found to contain small glass particles which could block the actuator and impact the functionality of the pump. Per Apotex, there is a potential for patients to be exposed to the glass particles and mechanical irritation cannot be ruled out. Local trauma to the nasal mucosa might occur with use of the defective product. Fluticasone propionate nasal spray, 50 mcg per spray, 120 metered sprays. NDC 60606-0829-1/LOT NJ4501 (7/2020)
On July 17,2018 Solco Healthcare issued a patient -level recall of valsartan tables. Please contact our office if you are taking this medication.
